Compensation & Benefits

• Initial Evaluation: $55–$65 per evaluation
• Follow-Up Visits: $44–$50 per completed visit
• Sign-On / Relocation Incentive: $2,000–$4,000, paid during the first 90 days

Position Overview

Care First Rehab is hiring a Physical Therapist (PT) to provide skilled rehabilitation services to adult and senior patients in their homes throughout Wake County.

This position is ideal for a therapist who enjoys working independently, building meaningful relationships with patients, and developing treatment strategies around the patient’s actual living environment.

You will evaluate each patient’s mobility and functional needs, develop individualized treatment programs, monitor progress, and work with patients and families to improve safety, movement, and independence at home.

Your Responsibilities

Patient Evaluation & Treatment

• Perform comprehensive physical therapy evaluations in patients’ homes.
• Assess mobility, strength, balance, functional movement, and daily activities.
• Develop individualized treatment plans based on clinical findings and patient goals.
• Deliver appropriate therapeutic interventions using current clinical practices.
• Track patient progress and adjust treatment plans when necessary.
• Complete timely and accurate clinical documentation.

Home-Based Rehabilitation

• Evaluate how the patient’s home environment affects mobility and safety.
• Identify potential fall risks and other environmental concerns.
• Recommend appropriate assistive devices and adaptive equipment.
• Suggest practical home modifications that support independence.
• Teach patients safe movement, transfers, exercises, and mobility techniques.

Patient & Family Education

• Explain treatment plans and therapy goals clearly to patients and caregivers.
• Provide families with appropriate strategies to support rehabilitation between visits.
• Encourage patients to actively participate in their recovery.
• Help patients develop confidence and independence with everyday activities.

Team Collaboration

• Communicate effectively with Occupational Therapists, Speech-Language Pathologists, physicians, caregivers, and other members of the care team.
• Participate in coordinated care planning when required.
• Share relevant clinical observations and patient progress with the appropriate team members.
• Contribute to a collaborative and patient-centered care environment.

Patient Population

You will primarily work with adult and senior patients who may require therapy related to:

• Neurological conditions
• Musculoskeletal conditions
• Balance and mobility limitations
• Age-related functional changes
• Reduced strength and endurance
• Difficulty with walking or transfers
• Fall-risk and safety concerns
• Functional limitations affecting daily activities

Qualifications

Required:

• Graduate degree from an accredited Physical Therapy program.
• Current and valid Physical Therapist license in North Carolina.
• Strong clinical assessment and treatment-planning abilities.
• Excellent communication and interpersonal skills.
• Ability to work independently during home visits.
• Strong documentation and organizational skills.
• Commitment to providing compassionate, patient-centered care.

Preferred:

• Previous home health or home-care Physical Therapy experience.
• Experience treating adult or geriatric patients.
• Familiarity with neurological and musculoskeletal rehabilitation.
• Experience recommending adaptive equipment or home safety modifications.
• Strong interest in community-based rehabilitation.
